What is a Care Specialist?

A Clinical Care Specialist (CCS) is an expert in all things cardiometabolic health. Your personal CCS has completed extensive education and training from accredited programs to give you individualized support and advice in managing your cardiometabolic health.

CCSs can help you reach your health goals, discuss everything from your blood sugars and medication to stress management and nutrition, or just be there to listen to you :)

    The cost of the Cash Pay subscription is $149/month. The following services are included in your subscription:

    1. Registered Dietitian Guided Intensive lifestyle management program – Comprehensive support for lifestyle changes aimed at weight loss.
    2. Medical provider consultations, when appropriate - If medications are applicable to your diagnosis, support is provided by a licensed medical practitioner.
    3. Connected weight scale – A weight scale that syncs with the program (with optional devices for other health needs).
    4. Custom meal planning (RxDiet) – Personalized meal planning to fit specific dietary and health goals.
    5. Lab testing – Included lab services to provide our medical providers with the most accurate blood tests (not billed separately).
    6. Insurance navigation – Help with navigating insurance, particularly in relation to prescriptions or other health services.

    Our monthly membership is designed to ensure we provide comprehensive and personalized care. While you may initially need a prescription, our Care Team will follow up with you regularly to monitor your health and adjust dosages as necessary.

    Please keep in mind that medication costs are not included in the monthly subscription fee.
